T3C Care and Private Information Agreement and Consent

Tokyo Community Care Center (T3C) is a volunteer operated and donation funded service seeking to provide quality and compassionate care. Because of the limitations in funding, and the focus of on-the-job-training of helpers, T3C has certain limitations to the level of care offered. Therefore, if you have any questions about the help offered at T3C, we encourage you to give us feedback and to seek a second opinion.

Should you experience continued emotional, mental, or physical problems beyond the scope of your care at T3C please see a health care professional as soon as possible. If you are feeling suicidal, contact a health professional for immediate intervention. You may inquire of T3C for assistance in locating additional help.

T3C seeks to offer the best care possible, however this does not warrant the accuracy, adequacy, or completeness of help offered. T3C will not be held legally responsible for difficulties that may occur during or after the care-giving process.

Every effort is made to guarantee your privacy and information, whether shared directly in counseling or provided to us through other communications. However, certain limited information must be disclosed when required by law or when deemed necessary to prevent injury to yourself or another party.

 Your voluntary submission to seek care service at T3C acknowledges your agreement and understanding of this document.

You may request of T3C the removal of any information held pertaining to you at any time.
